PT INR MOBIL CLINIC - "le coordinateur parle français"
The greatest drawback of mechanical valve is the need for life-long medication to thin the blood to avoid clotting of blood in the implanted valve and causing serious thromboembolic complications (such as stroke, paralysis, acute ischaemia of hand and leg etc.). That medicine, WARFARIN, should be taken seriously as well. In the case of overdose of it, blood will be too thin and it will cause other type of complication (such as bleeding in brain, abdomen, thigh, eyes etc.). Therefore the right dose of Warfarin should be maintained all the time. The monitoring of warfarin effect, that is how effectively each and every individual’s blood is thin enough to prevent clot formation, but not that much to cause internal bleeding is done through especial blood testing. That test is known as PT and INR.
Actually this program to check PT and INR is conducted in 21 clinics and 1000 patients with mechanical valve are registered in the process. Among them, 750 are coming regularly monthly in the clinics.

BY PROVIDING STRIPES: stripes are necessary to collect drop of blood from the patient. Stripes are available in India. Cost is about 375 rupees (nc) each., with
According to last census, 750 patients are regular coming monthly for PT INR check up. Then, we need 825 stripes monthly (750 +10% security in case of wrong manipulation).
Cost will come 825*375 = 3 09 375 nepali rupees per month.
BY PROVIDING MACHINES: The machines are necessary to control INR. Machines are available in India. Cost is about 50000 rupees (nc). Machine is: COAGUCHECK-XS TYPE 03666794001
BY PROVIDING MEDECINE: quest laboratory is providing free WARFARIN medicine for 10 days. We HUBLO are supposed to provide free medicine for other 20 days.
According to last census 750 patients are regular monthly. One patient need 4 rupees tablet per day, means 80 rupees for 20 days. 80 rupees per patient means almost nothing if we compare our European standing. But for 750 patients, monthly we need 750*80 = 60000 nepali rupees monthly to provide free medicine for the patients. Per year it means 60000*12 = 7 20 000 or 6500 Euros (1 euro/110 rupees).
